For information on service medals or records, visit the Government of Canada website. Depending on your situation, assistance for first issue or replacement medals is available through Veterans Affairs Canada for Second World War, Merchant Navy, and Korean War veterans, or National Defence for post-Korean War service.

Replace a Certificate of Service (CF 54) or a CF Service Pin

To request a replacement Certificate of Service (CF 54) or CF Service Pin, contact the Release Benefits Administration at National Defence Headquarters via email at CMP_DPSP_RBA_ABL_DPPS_CPM@Forces.gc.ca.

Follow these two steps to complete your request:
  1. Complete the Application for Records of Service, available here.
  2. Submit the appropriate fee:
  • $9.00 for the Certificate of Service
  • $10.00 for the Service Pin
Make sure both the completed form and payment are submitted with your request.
